Field Label	Description
Name	Enter your name in this field.
Print Email Address On Estimate	When marked YES, your email address is printed on the Estimate Report.
Email Address	Enter your email address in this field.
Print Phone Number On Estimate	When marked YES, your phone number is printed on the Estimate Report.
Phone Number	Enter the phone number where you can be reached.
My Job Function	Select one of the options listed. Options include: I write estimates I review estimates I write and review estimates
